The agreement that governs your use of this website and the assessment platform, written to be readable.
1. What quitOH.ca is
quitOH.ca is operated by 1553084 B.C. Ltd., doing business as Carebridge Pharmacy (College of Pharmacists of BC pharmacy licence 15176), 101-4484 Main Street, Vancouver, BC V5V 3R3. The site provides health information and a secure platform through which Carebridge's pharmacy staff review your file and arrange assessment of alcohol use with independent, licensed BC physicians and nurse practitioners at partner clinics, and coordinates optional dispensing and follow-up through Carebridge Pharmacy. Neither the website nor the pharmacy diagnoses or prescribes.
2. Not medical advice; no guaranteed outcome
Content on this site is educational and is not a substitute for advice from a clinician who knows your situation. Completing a screening or assessment does not guarantee a prescription or any particular outcome. All clinical decisions are made by the assessing clinician using their own professional judgment.
3. Emergencies
This service is not for emergencies. If you have severe withdrawal symptoms, thoughts of self-harm with intent, or any medical emergency, call 911 or go to your nearest emergency department. See Get help now.
4. Eligibility
The service is for adults 19 and older located in Canada. You agree to provide accurate, complete information, including about medications (especially opioids) and health conditions, because clinical safety depends on it.
5. Your account and security
Access to your assessment and dashboard uses secure links and authentication. Keep your email and phone secure, and tell us promptly if you believe your access has been compromised.
6. Pharmacy choice
Any prescription may be filled at any pharmacy in Canada you choose. Carebridge Pharmacy is an optional coordinating partner, and no incentive is offered for choosing it.
7. Fees
For BC residents with MSP, the clinician assessment is billed to MSP with no visit fee. For people elsewhere in Canada, or without MSP, a private assessment fee applies and is disclosed in full before you commit to it. Medication costs are separate and are quoted by the dispensing pharmacy before dispensing. See pricing and coverage.
8. Privacy
Personal information is handled as described in the privacy and consent policy, under BC PIPA and PIPEDA, with data stored and processed in Canada.
9. Acceptable use
You agree not to misuse the platform, including attempting to access others' information, submitting false identity documents, or using the service on someone else's behalf without lawful authority.
10. Intellectual property
Site content is owned by or licensed to the operators of quitOH.ca. You may use it for personal, non-commercial purposes. The AUDIT screening instrument is a World Health Organization publication used with attribution.
11. Limitation of liability
To the extent permitted by law, the operators of quitOH.ca are not liable for indirect or consequential losses arising from use of the website. Nothing in these terms limits liability that cannot be limited under BC law, and nothing changes the professional obligations of the clinicians and pharmacists involved in your care.
12. Changes
We may update these terms as the service evolves. Material changes will be posted here with a new effective date, and where appropriate you will be asked to review them.
13. Governing law
These terms are governed by the laws of British Columbia and the applicable laws of Canada.
